Clinton County Health Department (CCHD) Partner Nurse Program (PNP) connects local health care professionals in clinical practice to those providing community preventive services. In doing so, the Partner Nurse Program strengthens communication and collaboration across the healthcare system for better health outcomes of all residents of Clinton County.

A CCHD Nurse makes a face to face visit quarterly, providing local medical practices tools and resources that support high quality patient care. The PNP Nurse serves as a point of contact, familiar face and link to additional resources.
