Note that any submissions made using fillable forms by anyone other than recognized Fire, EMS or LE agencies operating in, or serving the citizens of Clinton County, will be deleted. 

When making use of these forms to submit data, if necessary, please feel free to print out a hard copy to gather the data at your convenience and then come back to the link/fillable form to enter the data to submit.

  • This form is intended for Fire Departments to submit/update agency contact information including Fire Chief, Line Officers, EMS Captain and Fire District Chair, Mayor or Incorporated FD President on an annual basis following elections or at any time there are changes in any of the positions or other data points. When submitting data sheet will go to OES Secretary, Fire Coordinator and Dispatch Supervisory staff.
