Expanded In Home Services for the Elderly Program (EISEP)
WHAT IS EISEP?
The Expanded In-Home Services for the Elderly Program is designed to provide non-medical services to older persons to allow them to remain at home.
HOW CAN EISEP HELP?
The three types of services provided under EISEP are:
- Case Management
- In-Home Aide Services
- Adult Social Day Care
CASE MANAGEMENT
The Case Manager assesses, monitors, coordinates and adjusts appropriate non-medical in-home services.
The beginning steps include:
Interviewing the person in his/her own home to identify needs. A financial inquiry is made to determine the amount of cost share. A service plan unique to each person is prepared.
IN-HOME AIDE SERVICES
In-Home Aide Services are designed to assist the client with non-medical needs: help with cleaning, preparing meals, telephoning, shopping and laundry, plus help with bathing, dressing, grooming, feeding, toileting and walking.
WHO IS ELIGIBLE?
Residents of Clinton County 60 years of age or older and needs help with :
bathing | dressing | toileting |
transferring | mobility | eating |
housekeeping | shopping | |
preparing meals | laundry |
and can be maintained SAFELY in their home.
Regardless of eligibility, resources are limited and entrance into program is not guaranteed.
WHAT IS THE COST FOR EISEP?
There is no cost for the assessment and case management aspects of the program. In-Home services may require cost sharing on a sliding scale basis.
